Therapy Success Story

After living at with two Home Health Aides coming into his home to assist him with his daily needs, Mr.C. began to have trouble getting up to walk. He was admitted to the hospital. The physician found that his Pace Maker was not working and he had another hole open on the side of the Pace Maker.

Mr. C. was discharged to Mission Care Center All Seasons on March 17, 2020 with decreased strength, functional mobility, transfers, ability to safely ambulate, reduced balance, functional activity tolerance, coordination, increased need for assistance from others, reduced Activities of Daily Living and pain. He received an Evaluation from Physical therapy and work daily to achieve his Goals. He stated, “You guys really helped me a lot.” He was discharged on April 7, 2020 with following achievements: Bed Mobility-from Minimal Assistance to Moderate Independence, Transfers from Minimal Hesitance to Modified Independence, Gait Training from Minimal Assistance 5 feet to 200 feet on even surfaces with a rollator, strength from3-/5 to 4/5 and standing from Poor + to Good. Mr. C. stated, ‘I am Happy to be here because I don’t think I could do it at home by myself. It was getting too hard.”
